Care Plus NJ Client Cloud Launches, Seeks to Improve Healthcare with Technology

The Cardinal Health Foundation recently awarded Care Plus NJ with funding to launch the Care Plus NJ Client Cloud project as a means of engaging patients in healthcare through the expanded use of technology.

The Cardinal Health Foundation offers these opportunities to healthcare manufacturers and patient care providers to help meet the demands of the evolving healthcare industry. The vision is to make healthcare more cost-effective and produce better patient outcomes, reduce prescription drug abuse, and create healthier communities.

The competitive funding opportunity that Care Plus NJ was selected for is called “E3 – Effectiveness, Efficiency and Excellence in Healthcare”. The expected outcomes of these efforts at Care Plus NJ include successful health management, improved medication adherence, greater health literacy and lower levels of inpatient hospitalization.

“Care Plus NJ is an excellent candidate for this funding because of our holistic philosophy and the integrated nature of the care that we provide,” explained Jessica Costa, Program Manager of Partial Care Services at Care Plus NJ, and Project Coordinator for the Client Cloud. “By putting technology into the hands of our community, this project will increase positive health and psychological outcomes and decrease social unfair treatment.”

In 2009 Care Plus NJ was selected as 1 of 13 organizations to receive four years of funding for Integrated Primary and Behavioral Health Care Services from the Substance Abuse and Behavioral Health Services Administration (SAMHSA). With that support Care Plus NJ was able to address the fragmented healthcare system, streamline medications, and enhance client compliance as well as safety.

The Integrated Primary Care Services paved the way for the development of a client-focused approach to care coordination and created more awareness about the important connection between a healthy mind and a healthy body. With the success of these innovations, Care Plus NJ was recently designated as the first Behavioral Health Home in the state.

The Client Cloud will begin with the selection of clients that are currently involved in the Behavioral Health Home and are either living in one of the Care Plus NJ group homes or supportive housing placements. Each of these clients will receive a securely managed Android tablet and will be trained to navigate the hardware and software as a tool for their health management.

“The introduction of this project was met with great excitement and commitment,” remarked Costa. “Technological opportunities have been few and far between for many of the individuals that we serve. Easy access to personalized health information and the monitoring of individual health progress will prove to be life changing for our participants.”

Each week Costa will conduct 5 1-hour training groups and each participant will be required to attend 2 per week. The goal is that participants will not only become more mindful of their decisions and responsibilities, but they will also have improved outcomes, involvement and public support activities.

Participants will have access to a cloud of tools and applications securely available through “mycareplusnj.org”, which will include access to their electronic health record. Some of the tools will be for health tracking, while others will be used as resources on topics such as primary and behavioral healthcare, as well as educational materials and general articles on wellness. Participants will also have a direct link to their health care provider through the tablets.

One other benefit to consider comes from a social perspective. As clients learn to use this new technology as a tool they may be able to improve past relationships with friends and families, and may even establish a new approach to peer support.

“The Care Plus NJ Client Cloud project has the potential to inspire change on many levels for participants,” concluded Costa. “Not only is it an opportunity for individuals to take a more active role in their health care, but it is also an opportunity to analyze their current lifestyle and begin to incorporate more healthy decisions and positive changes.”

In substance use recovery learning self efficacy (self-belief), challenging negative beliefs, creating a positive mindset, and investing in new values gained from addressing one’s substance use disorder and Behavioral Health issues – improves oneself, relationships, and the future.

Every patient at Care Plus NJ has access to Behavioral Health professionals, including psychiatrists, and psychologists. In addition, each is assigned a case manager, drug and alcohol counselor, and peer recovery specialist who play a pivotal role in the treatment process.

As a Behavioral Health professional, they guide education, group therapy and support groups and other Behavioral Health services as indicated based on each persons needs.

Motivation

Motivation refers to the internal drive, desire, and commitment of an individual to initiate and sustain positive changes in their behavior, lifestyle, and overall well-being. It is a crucial factor in the C.L.I.M.B. recovery process, as overcoming addiction often requires significant effort, perseverance, and a genuine commitment to change.

Motivated individuals in recovery set clear and realistic goals for themselves. Utilizing the stages of change model, Care Plus NJ walks patients through various stages of a substance use disorder, including precontemplation, contemplation, preparation, action, maintenance, and relapse prevention. Understanding these stages helps individuals navigate their unique recovery journey.

Insight refers to a deep and clear understanding of oneself, one’s behaviors, and the factors contributing to substance abuse. It involves gaining awareness into the root causes of addiction, recognizing patterns of behavior, and developing a clearer understanding of the impact of substance use on various aspects of one’s life. Insight is a critical component of the recovery process as it provides a foundation for positive change and sustained recovery.

Insight involves exploring the underlying issues that may contribute to drug abuse, such as past severe psychological distresss, Behavioral Health issues, or unresolved emotional challenges. Addressing these root causes is essential for long-term recovery.

Courage in the context of recovering from drug addiction, refers to the ability to confront and overcome the challenges and fears associated with the recovery process in a person’s life. It involves facing one’s vulnerabilities, acknowledging the need for change, and taking positive steps toward a healthier and drug-free life.

Love plays a significant and multifaceted role in the process of recovery from drug and substance abuse. Having a network of supportive and understanding family members, friends, or partners is immensely valuable during the recovery journey. Community, fellowship and support groups emphasize the importance of connection with others who understand the challenges of addiction. Repairing relationships damaged by substance use often involves acts of love, forgiveness, and rebuilding trust.

In essence, love in the context of substance use recovery encompasses self-love, support from others, understanding, accountability, community, rebuilding relationships, and a renewed appreciation for life.
