Care Coordination and Case Management
Navigating healthcare systems can be overwhelming. Our Care Coordination team ensures that no one has to do it alone. Care Coordinators serve as a central point of connection between providers, services, and community resources — they help remove barriers, streamline communication, and ensure continuity of care.

Social Support Services

Peer Support Services
We provide peer and family support services that enhance recovery, engagement, and overall wellbeing. Our Peer Recovery Specialists offer support outside of traditional outpatient care, complementing clinical services across the CCBHC model.

AOTS
Assisted Outpatient Treatment Services
AOTS provides intensive, community-based support for adults living with serious mental illness who have struggled to remain engaged in treatment or have experienced repeated hospitalizations, crises, or involvement with emergency services. Through a court-ordered treatment plan when appropriate, AOTS helps individuals stay safely in the community while receiving coordinated psychiatric care, medication management, therapy, case management, and connection to additional supports. The program focuses on improving stability, reducing hospitalizations, increasing treatment engagement, and helping individuals build greater independence and quality of life so they can achieve long-term recovery.

Referral forms must be completed in collaboration with a treatment team member and the treating psychiatrist.
CSC
Coordinated Specialty Care
Early intervention significantly improves long-term prognosis and recovery. If you or a loved one is experiencing unusual thoughts, hearing or seeing things that others don’t, feeling disconnected from reality, experiencing changes in mood, or socially withdrawing — you are not alone. Our treatment team works with those in the early stages of significant changes in mental and behavioral health.

ICMS
Integrated Case Management Services
ICMS helps individuals maintain stability and wellness in the community while reducing the need for hospital readmission. Our team connects individuals with essential community resources, builds practical skills for managing behavioral health, and works with them to overcome barriers so they can live as independently as possible.

Include: most recent psychiatric evaluation or mental health assessment, plus hospital admission and discharge documents if applicable.
IOTSS
Intensive Outpatient Treatment & Support Services
Also known as the Crossroads to Wellness Program, IOTSS is designed to meet the needs of adults residing in Bergen County with acute mental health issues. It provides rapid access to comprehensive services so individuals can maintain stability in the community and avoid hospitalization. Services are provided on a short-term basis for a period of 2 to 3 months, while an individual is assisted with linkage to community providers.

KAOS
Korean American Outreach Services
This program is specifically designed to provide appropriate services that meet the linguistic, cultural, and behavioral health needs of the fast-growing population of Korean American children, adolescents, and adults in Bergen County. Services help the Korean American population navigate behavioral health challenges with dignity and respect.

Partial Care
Partial Care Program
The Partial Care program is designed to support adults managing serious and persistent mental illness who need structured day treatment to thrive in their homes and communities. It specializes in helping individuals transitioning from inpatient care settings or those at risk of hospitalization by providing holistic psychiatric, medical, and social support so they can achieve healthier lives.
