CarePlus offers a wide array of programs and services to provide a holistic, comprehensive approach to aid in recovery from mental illness and substance abuse. If you or a loved one in the Northern New Jersey area need help, contact us for an appointment.
What to expect at your first appointment
- Your insurance or medical card and if applicable, established co-pay, as payment is due at the time services are rendered.
- Your primary care physician’s name, address, and telephone number.
- A complete list of all currently prescribed medications, prescriber’s name and phone, dosages, frequency, and the date you began taking the medication.
- A list of any prior mental health or substance abuse services you have received at CarePlus New Jersey or elsewhere.
- The name, address and phone number of someone to contact in case of emergency
- If this appointment is for a child, we will need proof of guardianship, copies of the most recent report card, IEP if there is one, most recent physical and blood work.
If you are under 18, you also will need:
- A parent or guardian with you for your first appointment.
- Parental or guardian consent for treatment.
- A parent or guardian with you for all psychiatric (doctor) appointments.
- Verification of billing information
- Consent to treatment form
- Information release form

Frequently Asked Questions
How long will it take to process my request?
While we make every effort to process all requests in a timely fashion, both the State of New Jersey and US HIPAA regulations grants us 30 days to respond to requests for medical records. If we are unable to complete your request within 30 days, we are required to notify you.
Please note:
- Valid Authorization Required – fully and accurately complete the request form provided on this site.
- Requests for medical records must be specific or the request is deemed invalid. Please choose either “Mental Health” or “Substance Use” or both for record type.
- The individual timeline to collate all client records is dependent on the level of care, duration and intensity of a client’s treatment, the number of programs in which the client has been enrolled, and the time period covered by the requested records.
- Third-party records cannot be released. Please contact the originating provider to obtain these records.
- State Mental Health laws require a clinical review of all records prior to release.
- Reasons for a delay in processing my request:
1. Clinical Review: A review to ensure nothing withing the record poses a risk to the client or anyone else. Also, if sessions involve another party or parties, we may require additional authorization for the participating party/parties.
2. Complexity of records: The number of pages, number of programs, length of treatment are all factors that would extend the review period, thus extending the processing time. - How do I know what is being released?
CPNJ will offer you the opportunity to review your records prior to release so that you can raise any questions or concerns you may have at that time.
