Paramus NJ- Care Plus NJ is pleased to announce a new program, Crossroads to Wellness. Crossroads is an intensive outpatient mental health service for adults with acute mental health service needs. The program provides rapid access to comprehensive and intensive services, while integrating a self-directed, wellness and recovery philosophy into its practice. It is intended to help adults who are being discharged from an inpatient hospital or other institutional setting back into the community by providing immediate access to community services during the transition. It will also benefit individuals experiencing acute mental health symptoms by providing intensive service options and additional supports to prevent psychiatric hospitalization.
The Crossroads team consists of Licensed Clinical Social Workers, Advanced Practice Nurses, a Recovery Specialist and Administrative Support. Services include individual therapy, medication monitoring, peer support, group therapy and family therapy, based on the client’s needs and recovery goals. In addition, existing case management and substance abuse services provided by Care Plus NJ will be available to the clients.
The program is located at Care Plus NJ’s main facility at 610 Valley Health Plaza in Paramus and is open to all Bergen County Residents. This service is made available through funding by the NJ Division of Mental Health Services. For further information call: (201) 265-1233.
